Wellness Checks

1-2 hours weekly
Social visit that could include vitals monitoring, basic health assessment (food/fluid intake, appearance, hygiene, medication compliance, infections or wound care)

I am here to provide a comforting presence and ensure that your loved ones receive the attention they deserve fostering a sense of security and well-being in their daily lives.

Initial Assessment

1-3 hours
A conversation about how I can best support you.

• • Initial Health Care Assessment and set up a care plan.
  • Advocacy Supports - cost dependent on duration
  • Phone call consultations - helping families connect with the right people and resources.
  • Support visits while in hospital - assisting with transitions ,family meetings, and collaborating with the care team.
  • Supports with issues in LTC/Retirement settings- who best to connect with

Doctor/Specialist Appointment Supports

Minimum 3 hours
I will attend Doctor appointments with Family/Senior Parent and ask questions, provide document explanations and provide debriefing session to explain or clarify information given in appointment.
***Will support family member getting on/off transport services but will not provide transport ***

Medication Assistance and Support

1-2 hours weekly
Fill up medication dosette to allow for compliance
I will support you to refill/reorder pharmacy medications
Can assist with Injectables such as Prolia, B12 and weekly diabetes such as Ozempic

Advanced Care Planning Session for Long term care /Retirement
Initial Consultation

1-2 hours
This includes:

• full health care assessment
• contact information for Ontario Health at Home (formerly CCAC)
• discussion about intake process
• assist at intake appointment, with home selection including tours and debrief sessions.
